HealthBridge UK – NHS Digital Health Companion App

Project Overview

HealthBridge Solutions partnered with NHS Bristol Integrated Care Board to develop a digital health companion addressing the GP appointment crisis — specifically the 40% of appointments consumed by chronic condition monitoring.

The Challenge

Diabetes, hypertension, and COPD patients attended routine GP appointments that could be safely managed remotely, consuming scarce GP time while offering no clinical improvement benefit over remote monitoring.

Our Solution

We built an NHS-integrated chronic care management platform to bridge primary care and patients:

• NHS Login integration enabling seamless, verified patient authentication without new account creation
• Vital signs logging for blood glucose, blood pressure, weight, and oxygen saturation with trend analysis
• Medication reminder system with adherence tracking and GP-visible compliance dashboard
• Secure GP and nurse practitioner messaging for non-urgent clinical queries
• HL7 FHIR integration connecting with NHS patient records for holistic care visibility

Key Results

• 12,000 active patients enrolled on the platform across Bristol and surrounding ICB region
• GP appointment demand for routine chronic condition monitoring reduced by 28%
• Medication adherence among enrolled patients improved by 41% vs the non-digital cohort
• Success led to national NHS contract expansion, deployed in 4 additional Integrated Care Boards
